Erectile dysfunction

Erectile dysfunction (impotence) is a sexual dysfunction characterized by the inability to develop or maintain an erection of the penis.
An erection problem is the inability to get or maintain an erection that is firm enough for a man to have intercourse. You may be unable to get an erection at all, or you may lose the erection during intercourse before you are ready. If the condition persists, the medical term is erectile dysfunction.
Erection problems are common in adult men. In fact, almost all men experience occasional difficulty getting or maintaining an erection. In many cases, it is a temporary condition that will go away with little or no treatment. In other cases, it can be an ongoing problem that can damage a man's self esteem and harm his relationship with his partner, and thus requires treatment.
If you have difficulty having or keeping an erection more than 25% of the time, it is considered a problem.
In the past, erection problems were thought to be "all in the man's mind." Men often were given unhelpful advice such as "don't worry" or "just relax and it will take care of itself." Today, doctors believe that when the problem is not temporary or does not go away on its own, physical factors are often the cause.
One way to know if the cause is physical or psychologic is to determine if you are having nighttime erections. Normally, men have 3 to 5 erections per night, each lasting up to 30 minutes. Your doctor can explain a test to find out if you are having the normal number of nighttime erections.
In most men, erection difficulties do not affect their sex drive.
Premature ejaculation (when orgasm comes on too quickly) is not the same as impotence. Together with your partner, you should seek counseling for this problem, which is usually due to psychological factors.
Male infertility is also quite different from impotence. A man who is unable to maintain an erection may be very capable of producing sperm that could fertilize an egg. An infertile man is usually able to maintain an erection, but he may be unable to father a child due to problems with sperm count or other factors.
An erection requires the interaction of your brain, nerves, hormones, and blood vessels. Anything that interferes with the normal process can become a problem.
Causes:
Diseases and conditions such as diabetes, high blood pressure, heart or thyroid conditions, poor circulation, low testosterone, depression, spinal cord injury, nerve damage (for example, from prostate surgery), or neurologic disorders (such as multiple sclerosis or Parkinson's disease)
Medications such as blood pressure medications (especially beta-blockers), heart medications (such as digoxin), some peptic ulcer medications, sleeping pills, and antidepressants
Nicotine, alcohol, or cocaine
Poor communication with your partner
Stress, fear, anxiety, or anger
Unrealistic sexual expectations, which make sex a task rather than a pleasure
"Vicious cycle" of doubt, failure, or negative communication that reinforces the erection problems
Erection problems tend to become more common as you age, but it can affect men at any age and at any time in their lives. Physical causes are more common in older men, while psychological causes are more common in younger men.
For many men, lifestyle changes can help:
Cut down on smoking, alcohol, and illegal drugs.
Get plenty of rest and take time to relax.
Exercise and eat a healthy diet to maintain good circulation.
Use safe sex practices, which reduces fear of HIV and STDs.
Talk openly to your partner about sex and your relationship. If you are unable to do this, counseling can help.
Couples who cannot talk to each other are not likely to be able to make love to each other. Men who have trouble communicating their feelings may find it difficult to share with their partner any anxieties about their sexual performance. If you keep worries to yourself, you are more likely to lose erections. Men who cannot express and resolve feelings of anger or frustration towards their partner may hold back their erection as a weapon in the conflict. In these circumstances, counseling can be very helpful for both you and your partner.

Although the drugs used to treat Erectile Dysfunction, Sildenafil (Viagra), Tadalafil (Cialis) and Vardenafil (Levitra), have generally been proven safe, recreational use of these drugs is associated with higher risks of sexually-transmitted diseases (STD's), including HIV. In a study in the current issue of The American Journal of Medicine, researchers from the San Francisco Department of Public Health evaluated 14 studies of Sildenafil use among men who have sex with men (MSM).
Sildenafil users engaged in unprotected sex with partners of unknown HIV status from twice as often to almost six times as often as non-users. HIV-positive MSM were almost twice as likely to be diagnosed with an STD if they were also Sildenafil users and the newly HIV infected were 2.5 times as likely to be users.
Further complicating the situation, 54% of users mixed Sildenafil with other drugs to enhance the sexual experience. One study reported that 36% of all Sildenafil users combined its use with other drugs, including methamphetamines (23%), ecstasy (18%), poppers (15%), ketamine (11%), and GHB (8%). A study among MSM seeking STD clinic services in San Francisco found Sildenafil to be used concurrently with ecstasy (43%), methamphetamines (28%), and amyl nitrate (15%).
Writing in the article, Jeffrey D. Klausner, MD, MPH, suggests that "Focused action is needed to target this ongoing public health problem. Although the current evidence of association exists only for Sildenafil, likely due to the relatively recent approval of Tadalafil and Vardenafil, the needed action should focus on all three phosphodiesterase inhibitors (PDIs), as the observed associations with Sildenafil use most likely would occur with drugs of identical nature. The labeling for PDIs should be modified to warn users of an increased risk for STDs, including HIV infection. Educational programs, particularly among physicians, MSM, and methamphetamine users, should be implemented to increase awareness as to the use of these medications and their association with STDs, including HIV infection."
In an accompanying editorial, Joseph S. Alpert, MD, University of Arizona Health Sciences Center, Tucson, is concerned about the public health implications when Sildenafil and its relatives are abused or used recreationally with illegal drugs. He points to a recent article published in The Lancet in which investigators from Rockefeller University reported on a patient who presented with a new strain of HIV virus that was considerably more virulent and progressed more rapidly to AIDS than any HIV virus previously found. Dr Alpert writes that "This patient had been abusing both Sildenafil and methamphetamine. The question that immediately leaped to mind was: What effect did the combined abuse of these drugs have on the patient? Could this have resulted in the development of the highly virulent HIV mutant? Does this combination drug abuse increase the likelihood of STD transmission and/or the propensity for unsafe sex?" Dr. Alpert also questions whether the risk of STD and unsafe sex is also increased in male heterosexual patients who use Sildenafil. He points to the need for increased physician and patient education as well as further exploration of the multiple implications surrounding abuse of Sildenafil.

The big news on the GOP side of the ball yesterday was the early departure of Fred Thompson and his followers, the self proclaimed "Fred Heads."  (Here).  Four whole months of campaigning got the best of Fred.  Leads one to wonder what four whole years of being president may have done to him.  His theory that you don't have to do any campaigning to win the nomination has failed miserably.  A true inspiration to slackers everywhere is no more.

If you saw Fred's speech Saturday after the South Carolina primary, you'd no doubt be thanking your lucky stars that Fred bowed out.  Could you imagine this guy giving the State of The Union speech?  I tortured myself by watching his speech Saturday and I swear it was annoyingly similar to a Grandpa Simpson rant.  It made me wonder if possibly one of the writers from The Simpson was drafting his speeches.  Is it against Union rules for members of the Writer's Guild to pick up freelance political work during the strike?  We may want to check on this. 

With Fred out, let's let bygones be bygones.  Besides, is he really out?  Don't kid yourself, Fred is likely on the short-list of potential vice presidential candidates.  My grandfather used to say that "you can't polish a turd."  But wouldn't the addition of Thompson to a McCain ticket be a little more shiny to conservative Southern voters?  ("Me like shiny things.")  Could the glow of a political marriage of Romney and Fred distract even the most intolerant evangelicals from Romney's worship of Satan, er, the Book of Mormon?  I guess only time will tell.  

In closing, we here at the PolitiPorn Editorial Board would like to wish Fred a fond farewell.  Freddy Boy, it's time to put the dentures in that cup next to your bed, leave them there for a long soak in the Efferdent and just relax while gumming a good Cuban cigar.

Don't worry Fred, if you don't get picked up as a VP on one of the GOP tickets, you could always become a spokesperson for Cialis.  Remember, Bob Dole did Viagra commercials with no back lash from the public or Libby Dole for that matter.  I figure most consumers would take a look at you and then look at your lovely wife, Jeri, and then hightail it to their doctor begging for a prescription.  Give it some thought, I'm sure the commercial shoot would only last 1 - 2 hours tops.  However, erections lasting more than 4 hours should be attended to by a doctor and despite urban legends to this effect, a trip to your local Hooters restaurant will do nothing for the condition.  The Cialis folks will explain it all in full detail.

i just unfortunately saw a swiffer commercial that angered me. the story goes something like this:

a woman is sitting on the stand in a court room. there's suspenseful music playing. people look angry. "can you point out the perpetrator ma'am," a young lawyer barks at her. "yes! right there!" she points to the defendant.

it's a broom. a fucking broom.

the defendant's lawyer stands and tears his glasses off his face. "objection!" he yelled. cue "one way, or another" theme song for swiffer.

snap to real life: i immediately grab my coat and head to the local grocery store to beat mobs of other people killing each other for a swiffer.

seriously, where do these marketing people come up with these incredible ideas for commercials? and who are the people saying, "yes, i like that way you're thinking, jones," and shelling out millions for such tripe?

will and i started a boycott list. on it are companies who pay for insultingly stupid commercials, absent of wit or creativity and riddled with predictable garbage selling tactics or blatant lies. swiffer just jumped into the top three, associated with the likes of the liberty mutual commercials in which people do nice things for each other (the music on one of them is ok, but c'mon you know insurance companies don't do nice things for us) and cialis, the viagra-like medicine in which people apparently relax in claw foot tubs void of plumbing but managing somehow to exist on a mountain top.
